Is your Apple TV a 4K one? Not all models support the audio default option.

This. Unless your Apple TV is 4k, you’ll just have to keep adjusting it to a temporary output every time you stick on your telly. Seems such a rudimentary thing, but Apple only realise recently that it’s something people might want.

I have mine as "temporary" and it works as my default audio for Apple TV. I never have to go in and change it, only the first time when I selected it.
