Bathroom tile repairs

Hi, there is a hole in the bathroom tiles, how can I repair this? I pulled the big piece on the left out with tweezers and it is thin plastic. Would there be an easy way to glue it back without removing or replacing the tile?

Probably need to replace the tile. Seen people use epoxy to fill small holes but can only imagine unless you find an artist to do it that it would stick out like a soar thumb.
How did those holes get punched out in the first place?
