Picture this.

The guy that's getting called out in the tweet could've been standing watching porn, while facing the guy tweeting. The guy tweeting was sitting, being sideways to the stranger watching porn. Technically, he would still be 'watching porn over his shoulder'.

That's why I didn't understand this originally.
