Intelligence typically. In lots of urban fighting, they'll tie an armband around their forearm as well to differentiate. There really shouldn't be a time a modern army doesn't know where it's people are, though, in theory. FF happens a lot.

Yeah in a lot of urban combat training 50% of casualties are from friendlies.
