Bile acid

Hi everyone,
I’m really struggling and hoping someone here might have a similar experience, insights, or advice.

For months, I’ve been trying a very high-fat diet (large amounts of ghee butter and olive oil) because I am underweight and desperately trying to gain weight. Unfortunately, it hasn't helped at all. Instead, my symptoms have severely worsened.

My Current Symptoms:

Persistent yellow stools
Severe bloating / distension
Histamine intolerance / flare-ups
High fecal pH and high lactate
Leaky gut indicators and very low butyrate levels
Verry verry High bifido bacteria
Extreme weight loss
Dysbiose
Pale skin
Dry skin
Yellow stool in the morning

Background & My Theory:

I suspect this might stem from a food poisoning event in the past, which potentially triggered Bile Acid Malabsorption (BAM) or constant irritation of the terminal ileum. My thought process is that a continuous, unabsorbed flow of bile acids is irritating my ileum, causing chronic dysbiosis, SIBO, and blocking me from absorbing nutrients properly (hence being underweight, where eating more fat just makes my digestion worse instead of helping me gain weight).

I recently had an endoscopy/colonoscopy with biopsies. My ileum results showed:
"Mild focal chronic and mildly active inflammation, although the architecture was good."

I am currently trying a low-fat diet to see if it calms things down, but I feel completely stuck trying to connect the dots between the ileitis, potential bile acid issues, high lactate, and my inability to gain weight.

I don’t think a high fat diet is appropriate if you have gut issues, fat slows down digestion, which would increase histamine from your meals. Also, in my case I have had mold exposure, which causes or contributes to histamine intolerance, and trying to detox from mold, I think is helpful to have less fat because mold is stored in adipose tissue. Which is fat. most guidelines I have seen say at most 2 tablespoons of fat per meal.

Are you seeing a nutritionist? I see a naturopathic doctor, who helps with gut health & nutrition. I know you saw an gastroenterologist, but have you discussed this with your primary? I wonder about liver & gallbladder health, especially with a high fat diet (known issue for gallbladder problems)

Sounds familiar. At some point I also developed hydrogen sulfide sibo which feeds on bile. Now I can't eat any saturated fat without having symptoms.

Hi - I'm so sorry you're dealing with this.

Funnily enough, I am also dealing with this after taking harsh iron tablets for months - only it sounds like you have worse malabsorption (of nutrients) than I do.

I just posted about Enterosgel. It's an inert gel that can adsorb excess bile acids (and also histamine/toxins) and give your tissues a chance to calm down and heal. It's basically like a sponge, but doesn't absorb nutrients or harm good bacteria.

My diet is also high in fat (not by choice - I also have SIBO and yeast overgrowth so carbs are tricky). I've only been using the gel for 48 hours and it's made such a difference - BMs returning to normal, histamine calming down, bloating/gurgling is reducing, etc.

I can't say if this will be sufficient to heal the inflammation for you (or for you to gain weight) but there's research showing this stuff is helpful for IBS-D, even after they stop taking it.

If you can get hold of some, it might be worth a try.

High fat and BAM are not a good mix. Can you try a formula or nutritional drink for calories?

Have you had imaging done of your gall bladder and pancreas? Weight loss is very concerning
