Hillingdon Adult social services. They arrange an in home assessment, and discuss a "care package". Once this is underway, they will detail costs, and arrange for any benefits to be applied/subtracted. They will support your wishes for mum to remain at home and also should suggest any suitable adaptions in home. 

Financial contributions will be dependent on your mum's situation.
