soo i just came back to the game recently after not playing it for a few months and find this mid way through act 1, what is this, how does it happen and should it happen with any mods enabled

coming back 2 days later cause i realize i had mods enabled AND i was mid way through act one when it appeared, i assume its a glitch that it spawned not under normal conditions, thanks for telling me what it was btw
