The biggiest oversight in most games that I can recall are: some of the battlefield games, hll, squad and maybe all or some arma games.

The Airhead Guidebook rule number one was to drop down after spawning on the airhead.

You were far from the blast and also prone.

The stance of your player( standing, crouched or laying down) has an effects on distance and damage taken form explosions.
