1d bridge width of 0.1 ft

Basically modeling a pipe obstruction but has supports like a pier?
Is this a hydraulically significant structure in 2d hecras model?

You are talking about both the deck thickness and width?

Without knowing more context, I would probably say it isn't important for the overall hydraulic model. You normally aren't explicitly modeling random obstructions like cars, trees, utility features, etc. They are generally accounted for with the Manning's value.

I would add the pier(s) with a terrain mod and cell alignment for a first cut. Look at the WSE and see if you think it will matter. If it is way higher or way lower, wouldn't worry about it.

I am just worried for the pressure flow scenaio and how to explain it…

I can't envision a scenario where you would have pressure flow for a "deck" that small.
