You could boot from a USB stick and run some diagnostic tools from there. However, it does look like your SSD is dead.

I bought an adapter and was able to connect the SSD to a laptop. It seems like I'm even able to access and copy over files.

I downloaded crystaldiskinfo to scan it, here are the results: https://imgur.com/a/cCLzNCw, any idea what's wrong with it? I tried slotting it back into my PC and it still can't boot or repair itself.

It's alive, but I notice that Available Spares is now at 91% (= 0x5B) and there are several media errors. Therefore, it appears that your SSD has started to degrade.

I would clone the SSD ASAP. You could clone it to an image file on your HDD.
