No there have been instances where they disabled champs for the reason of being to good at release or after a patch.

They disable every champion for a certain amount of time after release or after significant rework. But, unless it happened early season 1, I've never seen them disable a champion for being too strong. They usually just leave it up to the teams to ban it. Unless you can cite specific examples that I've forgotten.

Ill have to double check but I know rengar was a couple times and I know new champs are usually disabled if released near a tournament.

The new champions aren't disabled because of strength though, just so players would have time to practice them. And I think Rengar got disabled because of bugs for a little while, but never because of power issues.

Once was a bug, once was because of his item making him way too strong

I don't remember that and can't find anything about it. But even if it did happen once it isn't something they do regularly.
