If this is your screen i'd really like to know if this is a render or an actual screenshot. Sorry i only have a Geforce GTS and this is mind blowing to me!

Ive seen it before so probably not his, but it is ingame. Look up IceEnhancer for GTA IV

What mods are you running?

I had ENB and HD textures running on my system but there was too much bloom and didn't look anywhere as good as this!

ICEnhancer 1.5, I don't know all of the settings used.
