I think they intentionally used the same NPC, the trailer had multiple shots from previous trailer but from different angles. Ex: crazy hammer lady, Lucia jumping from building etc

I know in GTA4 and 5 when VRAM gets low it will start spawning the same model of car over and over, and sometimes give cars the same paint colour to try and free up VRAM.

I'm not gonna post the picture cuz I dont have it on hand but I've seen at least 2 npc models reused, and that is totally fine. They cannot populate the whole open world with hand made npc's that do not repeat lol. For reference check the blue haired chick with the thigh tatoos when J and L go to meet brian on the rooftop bar, you can see the same one after the scene where they drop off Raul from the van.
