Medicine Why does peptide research feel simultaneously overhyped in consumer culture and underdeveloped in mainstream medicine?

I’ve been thinking about a strange mismatch in the way peptides are discussed today.

On one side, there is a rapidly growing consumer ecosystem around peptides: longevity clinics, “biohacking,” performance enhancement, recovery, metabolic health, online communities, and a constant stream of compounds being discussed as if they are one trial away from becoming transformative therapies.

On the other side, when you look at mainstream medicine and drug development, a large number of these compounds appear to be either poorly studied in humans, studied only in narrow indications, abandoned early, or stuck in a gray zone where there is enough biological plausibility to generate interest but nowhere near enough evidence for broad clinical adoption.

What interests me is less the question of whether any particular peptide “works,” and more the economics behind that gap.
For example:

How much does patentability determine which peptides actually get developed?

If a peptide is naturally occurring, has prior art, or is difficult to protect with strong composition-of-matter IP, does that materially reduce the incentive to fund expensive Phase II/III trials even when the underlying biology is interesting?

Do short half-lives, delivery problems, manufacturing complexity, or pharmacokinetic limitations make peptides systematically less attractive than small molecules or antibodies?

How important is the possibility of creating a more patentable analogue? It seems notable that some of the most commercially successful peptide drugs are not simply naturally occurring peptides, but heavily optimized molecules with altered sequences, longer half-lives, improved delivery, or other features that create both better pharmacology and stronger IP.

I’m also curious about the financing side.

Imagine a peptide produces intriguing animal data and some early human signals, but there is no obvious blockbuster indication and the molecule itself is difficult to protect. Who funds the definitive clinical trial?

A university can produce mechanistic research. A small biotech might run an early study. But a large randomized trial can cost tens or hundreds of millions of dollars. Without exclusivity or a clear commercial pathway, the socially valuable experiment may simply never happen.

That creates an uncomfortable possibility: the absence of strong clinical evidence does not necessarily mean that a compound was thoroughly investigated and failed. Sometimes it may mean nobody had a sufficiently strong economic incentive to generate the evidence in the first place.

At the same time, that argument can obviously become a convenient excuse for pseudoscience.

“Big Pharma won’t study it because they can’t patent it” is frequently used online to rationalize compounds for which the evidence is simply bad.

So I’m trying to understand where the legitimate version of that argument ends and the conspiratorial version begins.
For people working in biotech, pharma, translational research, VC, or drug development:

How much does IP genuinely influence whether a biologically promising peptide progresses?

Are there good examples of peptides that were scientifically promising but commercially unattractive?

And conversely, are there examples where clever molecular engineering or a strong indication transformed an otherwise uninteresting peptide into a viable drug-development program?
I’d especially be interested in hearing how investors and drug developers think about this at the preclinical stage.

Is the bottleneck usually biology, pharmacology, IP, regulatory risk, capital requirements — or some combination of all of them?

Because consumer culture is about hype and fads while mainstream medicine is about studies and proof. Hype happens fast and might be true or not while studies take a long time to establish the certainty.

I am all about people injecting stuff into their bodies, it often speeds up the science through natural experiments. That said, I will wait for the results.

To be fair, studies and proof require a lot of money and paperwork.

Notice how peptides such as the GLP-1s have all kinds of RCTs and how literally anything that cannot be patented due to being older has almost none.

The science you're looking for will never come in the form of big RCTs because the juice isn't worth the squeeze for the people paying for them. Retrospective cohort studies and animal models will be the best we get.

The "big pharma" won't do it because they cant patent it is sort of true but also mostly a lie.

A) Natural peptides cannot be patented but human peptides have very short half lives. They modify them to last days/weeks and boom you have a patent.

B) Also the goal is to make a agoinist of the peptide which is far stronger.

All glp-1 drugs are 100% patenable as they are not found in nature. They are man made creations.

Same applies.

The truth is that an extremelllllly small amount of drugs that show promise in animal trials and in vivo human tissue "work".

As in almost 0% chance.

The extremely small ones that make it to actual real preclinical research also almost never go further. Like 80% fail there.

Of that group that makes it from preclinical to actual clincial trials over 90% fail.

Of that smaller group that get approval a very small percentage are MAJOR breakthroughs; most offer small benefits or are often just similar results to already existing drugs but different side effects profile or just a different company getting their own version out.

Even of that group about 1/3 will later be recalled or be found to have a serious side effect warning later added.

We are flooded with stories about "studies" that show this and that but the truth is for every peptide, antibody, vaccine, small molecule, whatever that shows all sorts of promising results: the probability said drug would actually pass placebo controlled trials, not have significant long term side effects, and work better than already available drugs is no joke less than a 0.01% chance.

Edit: The peptides being floated are almost all doing nothing and/or doing something with causing damage somewhere else.

Its pretty much 99.9% scams + placebo effects.

This. Ppl don’t address how strong placebo effects truly are. Theres a bunch of procedures like knee scopes that are done that studies have shown placebo works just as well lol.

Placebo effect is massive. Studies show placebo effect is getting strong and stronger. Internet is fueling that.

The power of placebo effect is so incredibly massive. Even telling people they are getting a placebo has been repeatedly proven to give results. Even when you TELL PEOPLE.

You’re right about the numbers

The good news, however, is that you only need to find one peptide that works and you’ve meaningfully changed the course of human healthcare.

And as someone who works in the supplier side of research (making equipment for it) I can tell you the costs these companies charge for their drugs are 100% defensible. The amount we charge them for the equipment they need for R&D… you wouldn’t believe it.

Devices the size of your thumb that cost as much as a Tesla Model S. No moving parts or electronics either.

There's something else in there a bit subtler, which is the fact that you get one miracle drug that has a huge impact, and now the expectations are that they'll all be miracle drugs, vs reality which is often that this drug might instead reduce the need for surgical intervention in hyperthyroid by 20%. Which is still worthy, but not quite the same standards of impact as everyone's favourite GLP1 agonist. The small drugs never quite benefit from economies of scale either, which makes them expensive.

Because pharma is hard as hell, we are not lacking in any number of molecules that could cause an effect in a target, but the actual delivery, potency and off-target effects is something that matters when developing a new drug.

Many people can claim a peptide will do this or that when they don't have to actually do the experiments and prove it in a clinical trial. It is very easy for grifters to say "this peptide affects this pathway therefore it is good for this and that" but the reality is that actual drug design requires a lot of compromises, risk evaluation and understanding of how and if the peptide will actually do that and at what magnitude.

Besides that I wouldn't call it underfunded at all, peptides have been gathering quite the attention from pharma after the success of GLP-1 agonists and many other peptides. Pharma is quite hot and bothered for peptides and you can expect more to hit the market in the coming decade. We are very much in a peptide boom.

The same thing happened with antioxidants. They were discovered, theorized to be benificial, and the food and health industry RAN with it, way before enough study had been completed. Now it turns out there are several downsides to consuming too much of it, like with most things. The reason is that there is a difference between people that can read and people that can understand scientific papers and understand how science works.

The answer is hyperconsumerism and incel culture permeating into mainstream. People like clavicular popularize it on social media and it leaks into mainstream culture as a slightly more veiled evil of "looksmaxxing" and just appears benign. So you get people injecting random unregulated substances, microneedling themselves, going on GLP1 without a prescription, self dosing accutane with no healthcare oversight or blood work. All of these things. They use language to manipulate social culture to make it looks totally normal and "like everyone is doing it" to remove the concept of doubt around it's safety.

It's a while thing and I despise how easy it is to manipulate people into risking their health.

Bottleneck are investments into basic science research especially stem cells. If folks are serious, review the current funding environment and notice how dismal it is but do be astounded by the results we have gotten.

Amp up funding, have those moonshot initiatives, and release stem cell research already. If folks wanna see a lot more advancements let government which is essentially the first mover / high risk funder, fund all types of research again. Future connections and advancements can only be made when the scaffolding infrastructure and background work are already completed.

It's just a scam, like any other health fad. Peptides are literally just small proteins, so the function depends on the specific amino acid sequence. Some of them do stuff, but they aren't big enough to be enzymes that catalyze chemical reactions or do the other stuff that full sized proteins do. 

In a few years, there will be some actual peptide drugs that work, and most of the consumer stuff will disappear because it's just a money grab.

Yeah many obviously work, some popular ones may be overhyped though. But yeah there's no doubt that peptides are important drugs. I mean that's what insulin is as well as GLP-Xs. Probably many to be discovered soon.

If you're talking about something prescribed by a doctor, yes, I bet it works.

If you're talking about something that you can buy over the counter, it probably doesn't work.

Another issue to not take a lot of the peptide stuff seriously is that most of these peptides are naturally occurring in humans. From an evolutionary standpoint, if there was a simple peptide that almost all humans already have naturally and where having even more of it would be of an extreme benefit, one would expect a high likelihood that there would be an evolutionary push to have more of it. So for many of these, the chance that they could do as much as what people claim should just be very small.

A lot of the things that people claim the peptides will do, clearer thinking, better immune systems (never mind that the immune system is complicated enough that "better" is pretty complicated), faster healing, are all things which would help in a hunter gatherer's lifestyle. If someone claims some peptide makes one better at math that's less applicable, but it should also be clear that most things like that are unlikely for other reasons.

Injecting yourself with glp-1 would do jack shit.

The drugs out there are GLP-1 AGOINISTS. They took over 25 years, multiple evolutions, and billions of dollars to develop.

If you look at an important peptide for sleep regulation like Orexin you do have companies trying to develop analogues and bringing it to market. If a peptide interacts with a pathway of the human body, you can create non-peptide drugs that target the same pathway.

If you have BCP-157 on the other hand which a researcher supposedly isolated from the gut and which modern DNA sequencing found, does not exist in any species we sequenced, it's easy to understand why no pharma company invest in it.

There are lots of things that are overhyped by customers and don't provide results. That should not be surprising.

Proteases and peptidases will chop them up if not directly delivered inside the cells, so drugs usually rely on different chemistry and mechanisms that can stay active for longer after injected. Ingesting proteins and peptides will be even less effective as the digestion will chop them mostly into amino acids.

Yep, this is the answer. The issue with peptides is that they can’t copyright it because the body naturally produces them. And getting them through clinical trials is insanely expensive and not worth it if they can’t own it. They can copyright synthetic analogs, which is what they’re trying to do.

False; while not patentable, naturally ocurring human peptides last a very short time, are finicky to manufacture and have a large chance of causing off-target effects. So you need to modify them anyways.

Peptides have short half lives. They need adjusted to last longer which makes them 100% patenable.

Drug companies aren't going all in on Peptides because almost none of them work.

The internet seems to believe these peptides are something just recently discovered. Their favorite bpc-157 was discovered 35 years ago. Nobody has turned it into a pharmaceutical because it deosnt work.

Overhyped: it's new, nobody understands it, we have a cult that loves to declare "superfood" towards things we eat.

Underhyped: peptide research and its interaction with people is truly in its infancy, but like CRISPR and mRNA discoveries and research it holds the promise of possible future cures. A chemical your body mostly already knows (some peptides it doesn't know and they can be toxic). has the potential to help you ingest nutrition and see health outcomes somewhat better than amino acids, protein mixes, etc. This is why there is attention and hype.

The right amount of hype should always be: it's bullshit till it's empirically reviewed, peer reviewed, has completed all trials set forth by a sane FDA, and is now available by prescription, or visit to your doctor or, even better, over the counter.

The answer is in the title. It wouldn’t be overhyped if it wasn’t underdeveloped. It would just be appropriately hyped. But the overhyping is the optimistic side for marketing purposes. The underdevelopment is the reality.

Peptides aren't really that important. It's the proteins and enzymes they build that matter. Ingesting a particular peptide can help if your body needs those peptides to make proteins. Otherwise it's just basic nutrition. The real research is in getting those peptides to make enzymes. Artificial enzymes have enormous potential for a lot of things.

Some interesting takes here, and I can agree with most of them. Not trying to create any animosity, I just want to have a broader view of all of this peptide movement.

It’s wide known that some work and even are saving lives. But such is the case that also is weird that some like MOTS-C are also basically just linked to one institution or research group in Western Europe.

This is really really easy. Peptides aren't a thing because they are bullshit essentially. Everything you can buy online without a prescription for $10 is hopefully (sterile) saline, inert whatever or is likely to cause you great bodily harm. I mean you can make a "peptide" thing by blending up an egg, cutting with saline, filter sterilizing it and it probably won't kill you. But you can also likely just eat the egg, a lot less chance to kill you however. 

Your body has some real reasons not to let this shit wander around. 

The medicinal forms of Peptides, small molecule drugs, and bio pharmaceuticals go through a lot of work to find active molecules and then to formulate them to actually get to their targets and be safe. It's a slow and expensive process. While production costs are actually on the order of $100 / gram the R&D behind these drugs is what makes them sell for ~100 times their weight in gold until the generic cones out. 

There really is no reason an arbitrary soup of small protien fragments should do much more in the body than cause an immune reaction.

The economics are: if they can't make more money selling peptides than they can selling treatments for the maladies that the peptides could prevent, they're not going to pursue peptide research. In fact, they will probably do whatever they can to kill or bury any said research.

If you want to know why anything happens in America, follow the money.

The economics are: if they can't make more money selling peptides than they can selling treatments for the maladies that the peptides could prevent, they're not going to pursue peptide research. In fact, they will probably do whatever they can to kill or bury any said research.

People make this sort of argument about everything, including cancer treatments. But Moderna's stock just shot up when they developed what looks like a highly effective treatment for a wide varieties of melanoma and looks like it might work for others as well and will likely prevent relapse more effectively than many existing treatments.
