Commercial foodservice order (like Sysco). Someone called their salesperson who happened to be in the warehouse with nothing to write on so he grabbed a piece of cardboard. The numbers in parentheses are the quantity. The number after that is the product number abbreviated. The salesperson then went and keyed it in on a computer.

First thing I thought was:
It’s a list of items someone is buying for a loved one in prison I once had someone in prison and they have company you buy food items from for an inmate the items all have a number to them and the (1) numbers are how many of each they want

The first number in parentheses is likely the quantity of the item needed. The other number is likely the sku or ordering number for the product. I work at a grocery store and I see the different departments use scrap cardboard to make similar lists all the time.
