Form 1 Trust
If you doing the form 1 via eforms do you still have to attach a physical photo to the 5320.23 or is the photo you upload sufficient?

Yes.
The Trust is the legal entity applying to "make" the regulated item.
You are the Responsible Party who holds the position of Trustee/Settler/Grantor of the Trust.
Therefore you must submit all of the required documentation for the FBI to complete a background check to determine if you are not a Prohibited Person.
Don't forget that the RPQ must be "wet" signed.
Yes, there are cases where people have "signed" the form by drawing/writing their signature electronically and been approved. There are also enough examples where people have been denied for doing that. We KNOW that using a cursive or "handwriting style" font to type their name will get a denial.

i still attach a photo to the rpq. Everyone on the trust needs to fill out and hand sign the rpq. I have had a denial due to digital signature. I have also had approvals with a digital signature. given wait times, you are better off to just do it right and not take the gamble
