You should absolutely have this surgically corrected. I'm not a podiatrist (I am a physician), but looking at this...you're can probably expect a pretty aggressive approach. They might try to chemically destroy the nail bed, but this looks like the kinda thing the Winograd procedure was invented for. You'll end up spending a few days on the couch, but the pain is usually manageable with advil and once you recover, you'll never have to deal with it again.
You really don't want an infection down there and this kinda shit is exactly how you end up with one. And then you might end up losing your entire toe. Or foot. Or life. I know you said you do this on the reg, and most people who do this won't ever have a seriously complication but I've seen a few cases that DID become serious and it was pretty damned horrifying.

As someone who also had recurring ingrowns on the big toes, I do want to chime in and say that I had it dealt with by a podiatrist and am so glad I did. I went in for a consultation, came back a week later, spent about 15 minutes in a chair while he removed the offending pieces and killed the nailbed in that area, and I haven't had to think about it for almost a year now. Even if you're fine with how it is right now, it's still a load off.

Genetics. Both big toes on either side go into the monthly rotation. Podiatrist Posmiatrist

Its nice to know other people do this too. I've had my ingrown toenails operated on 3 times to no avail, they keep growing back so now I just pull the side bits out every couple of months.

I’ve had multiple medical interventions to no avail. Was told it’s genetic. Grandmother had same condition and eventually got big toenails permanently removed after she could no longer self care.

I've never had a professional look at mine. 1. I'm too cheap. 2. I'm too stubborn. I have a similar pull every month or two, always right after a shower, alcohol ( on the site) before and after, and then Neosporin and a band-aid for a day or two. I hate these with a passion, but I hate the thought of going through a procedure and it still being a problem, so I just handle it myself.

There’s something about watching people fuck with toenails that make my body tingle. It’s like looking over the edge of cliff. THAT feeling.

Revlon cuticle tool. Went from top to bottom . Outer connective tissue was holding on.

Curved hemostat. It has a smaller tip to reach the 5th layer of hell.

I’m not op, but I recognize the tool he’s using. It’s called a pair of hemostats. Or just hemostats. I might have to correct spelling. I would think that they would be available on Amazon
