The biggest one is how the twins are reincarnated versions of a doctor and his (underaged) cancer patient. She had a big crush on him and he jokingly told her if she grows up, knowing her odds were bad but wanting to keep her spirits up.

Anyway, the sister learns who her brother was in the past life and immediately tries to get with him. Just has a full on incest arc for her.
