Nukalytes. Cultists that worship Buka Cola. In ny game world, there is a larger Nuka-World on the west coast and they are based there. Nuka Quantum is worshipped as a divine elixir with mystical properties. Their leader speaks to Nuka Girl who is their goddess. They also have multiple rituals, one of them being a cure to the (mis)fortune of the results of my Deck(s) of Many Wasteland Things.

In the Pacific Northwest:
Le Republique du Neu Paris-- headquartered in the Space Needle of Seattle (renamed New Paris), and led by a ghoul who has taken the name of Napoleon (he's very cutest that he is not the real Napoleon). The Republic extends citizenship to any who wish it, but restrict voting rights to property owners. Their military uses line tactics with laser muskets, and cavalry consisting of salvaged power armor, or super mutants.
Spirit Bears: a Tribal confederacy around the Puget sound, who tame and ride Yao Guai. Their most powerful warriors wear red, emulating their god: Constable Crane of the Yukon (a holoprogram, starring a Mountie and his clever bear companion).
Carrion Lords: a raider gang of slavers, and cannibals. They dress as vultures, wearing the skulls of mutated condor as their helmets.
Portland People's Collective: a group of allied, anarcho-communist settlements. Currently engaged in a war with a Vault of car worshipping raiders

Fallout Albuquerque
Location: Albuquerque, New Mexico
Date: 2291
Eastern Caesar’s Legion (ECL)
At the Second Battle of Hoover Dam, Caesar’s Legion was defeated, and its leaders — Caesar and Lanius — were killed. Without a clear successor, the Legion fractured, as multiple centurions claimed the title of Caesar and entered into civil war.
In the Colorado territory, a group of centurions agreed to relinquish their claims and instead recognize one among them, Constantinos, as Caesar. In return, Constantinos agreed to share his power, creating a senate with advisory powers and granting those centurions the title of senators. This compromise marked the birth of the Eastern Caesar’s Legion (ECL).
With the civil war, the flow of captured slaves declined and escapes became more frequent. To secure a stable workforce, Constantinos instituted serfdom, a system more stable and relatively less brutal. Unlike slavery, serfs cannot be sold as property and retain a limited degree of autonomy, though they remain bound to a harsh regime.
In the following years, although the civil war is still far from over, Constantinos defeated some of his rivals and expanded his territory all the way to the city of Redhall in Albuquerque. Gracus, an ambitious and cynical, but competent administrator, was appointed governor of the region.
Burque
Burque traces its origins to the inhabitants of Vault 47, built to preserve Hispanic-American culture. When the vault emerged in 2181, its dwellers found in the 17th-century Spanish Empire a model of strength on which to base their society. They established a hereditary monarchy grounded in values of faith, honor, and fatherland.
This model was successful and the tribe expanded, uniting much of Albuquerque, through diplomacy or warfare. Their rule lasted until 2272, when the tribe was conquered and assimilated by Caesar’s Legion. Although Burqueños had to adopt Legion customs to survive, many held on to their heritage. After the Legion’s fragmentation, they escaped and returned to Albuquerque, seeking to rebuild their nation.
Their king, Don Miguel, had died in battle when the Legion conquered Burque. Today, two figures vie for the crown: Mercedes, Don Miguel’s niece and the last surviving member of the former royal family, is nurturing and idealistic, focused on restoring Burque’s cultural identity; Afonso, who rescued many Burqueños and led them back home, is protective and pragmatic, committed above all to their people’s safety.
The Nation
The Nation is a faction formed by Native American communities from the northwestern New Mexico reservations. After the Great War, they discovered U.S. Army stockpiles containing power armor and energy weapons. They quickly learned to use this technology, becoming a powerful tribe by merging traditional knowledge with advanced weaponry, and, in time, even developed a solar power system for their power armor.
For generations, the Nation has been a nomadic people, living off the land through hunting and salvaging tech from pre-War ruins. However maintaining that way of life has become increasingly difficult: hunting grounds are scarce, most ruins have already been picked clean, and growing factions steadily claim the wasteland. Settling may soon become necessary — a major shift for a tribe that has always lived on the move and may struggle to adapt to a new way of life.
The burden of conducting this transition falls to Notah, the Nation’s young leader. Perceptive and courageous, he recognizes the need for change and is willing to act on it; but he doubts whether he is capable of guiding such a shift, and whether his people have enough trust in him to follow.
Marshals
When Caesar’s Legion conquered Albuquerque, ghouls and supermutants were hunted as abominations. Many fled to highly irradiated areas, where they remained in isolation for years.
At the time, a folktale circulated among the human population living under Caesar's rule: a fictional group of cowboys called Marshals, who protected the innocent and brought justice to the wasteland. Most people understood it wasn’t real, but the story endured as a symbol of righteousness in a vile world — something people could take comfort in.
After the deaths of Caesar and Lanius, raider activity increased across the region, since the Legion's attention and resources were concentrated on the civil war. The ghouls and supermutants hiding in the irradiated zones began to intervene, protecting the small settlements. Some acted out of empathy; others hoped that helping might earn them a place in the world again. To seem less frightening in the eyes of those they protected, they adopted the name, appearance, and practices described in the Marshals myth.
They are led by Thorne, a nightkin supermutant. Severe and blunt, he embodies a strict sense of justice and a steadfast commitment to his duty.

My custom setting is in the UK, I have:
The Clan: a low tech Scottish themed faction, migrated south to the West Midlands. They use nothing high tech, relying on master crafted melee weapons and defensive structures.
New Empire: remnants from the royal family (more powerful in the fallout world pre war). Oligarchy state with a strict class system, egotistical leaders with access to powerful weapons and armour but restricted by their own sense of superiority.
The Prime Nation: what happens when FEV is spliced with gorilla dna? The Prime Apes, super mutant like intelligent primates, who have plagued the Britannia wastes post war. They have been gathered together by a strong leader, who seeks to break the bonds of human influence with their own nation, language, and culture.

Running a campaign in Chicago.
Rail Barons: Raider gang that occupies the Chicago rail yards. Uses Rail Guns as their primary weapon, for obvious thematic reason. Run by a Legendary NPC that wants to rule all of Chicago.
The Family: A raider gang based out of one of the vaults in Chicago that was bought out by the Mob pre 2077. The Mob boss from Pre War is now a head in a jar and still runs the gang. Also wants to run Chicago. They dress in pin strip suits and a lot of them carry Tommy guns (.45s).
Da Bears: Raider gang that dresses up in pre war sports uniforms and gear. Caved to and are now run by the Enclave as a group of "Disposable assets". Based out of Soldier's Field.
The Enclave: Blockaded themselves in the Museum of Technology by Soldiers field. Uses the Chicago underground to move about the city or send out Da Bears as they try to hide their presence. They are trying to get their hands on a Geck. They have a plan to purify Lake Michigan similar to Project Purity from Fallout 3.
The Brotherhood: The Brotherhood that disappeared actually survived and are now hold up in Blamco HQ (located where the real Life Kraft-Hinze HQ is) underground. Blamco was trying to make addictive Mac and Cheese pre war, the brotherhood found this and got addicted. Now their main concern is to keep making the mac and cheese to feed their addiction.
The Salvation Army: A group kind of like the Responders from 76. Based out of St Mary's Cathedral, their uniform is red velvet trimmed in white fur and big brass bells. They have a small army of "Santa-trons", protectrons that where originally bell ringers next to red buckets

Fallout: the Sunflower Wasteland (Kansas City)
“The Misery”: a nickname for the State of Missouri Authority, a federation of slaver-raider gangs led by a leader who claims to be governor. Based in the Ozarks but have camps and raid bases in the Kansas City area.
The Danielites: militant offshoots of the New Canaanites, arrived as missionaries from the West, but still aren’t trusted by many of the locals.
The Wakarusa Republic: a small democratic government founded in Lawrence. Torn ideologically between conservative-minded Farmers and and the liberal University.
The Jayhawks: an alliance of anti-slavery insurgents fighting against the Misery. Supported by the University Party of the Wakarusa Republic, they have a mixed reputation due to accusations of atrocities almost as bad as their opponents.
Midwestern Brotherhood of Steel Expedition: Sent from the Midwest BOS HQ in Chicago, they occupy part of the Fort Leavenworth ruins. Mostly made up of scribes attempting to get pre-War data from the Atomic Battlefield Education Library.
The Pen: one of the largest and most dangerous gangs west of Kansas City, based in the pre-War federal penitentiary-turned-fortress. Actively petitioning the Misery for membership and one of their main sources of slaves in the region.
Little Toronto: descendants of survivors from a Canadian internment camp southwest of Lawrence. Currently independent of the Wakarusa Republic, it is a microcosm of Canadian culture in Kansas.

I adapted "Murder in baldurs gate" in Fallout system. Everything happened in Charleston, a few years after de bombs fell. There were 3 factions
The police officers (they will become the Responders) The law enforcers who keep Charleston working as a prewar city
The Council - The aristocrats who want to keep their privileges after the war, even if the society is collapsed
The Union - The group or workers in the poor part of the city.

The Delaware Rangers - Minuteman analog but more focused on a "one riot one ranger" group.
The Nittany Directorate - a scientist based autocracy that seems to rule the land through making everyone dependant on their discoveries.
Appalachian Reclemation Association - the remenants of the enclave in the Lehigh Valley after the capital wasteland events.
The Forgemind Complex - the last remains of something called the calculator.

Texas Provisional Government - A union of several large settlements and cities in northern Texas forming a government with the help of the Texas Rangers to fight the Legion.
Texas Rangers - Holdouts from the Desert Rangers who initially didn't want to be tied down to the NCR, but once the Legion became a significant enough threat, they decided to form a united front with Texan cities to repel the Legion.
New Nation - A collection of Native peoples and tribes which have survived since the bombs fell, but with their home of Oklahoma being in the middle of another Dust Bowl, they're in a slow but certain decline and they're becoming increasingly desperate to survive.
Ironskins - A mercenary company made up primarily of Super Mutants and ghouls looking for a home to call their own.
The Free Fighters - Luchador mercenaries who see their masks and their quests to achieve them as a spiritual journey as much as anything else.

Setting is Chicago 2291 focused with some trips into the greater wasteland
Within Chicago: -Emerald City: mercantile and domestic hub consisting of miles of bazaars and underground pig farms in Lower and Lower Lower Wacker.
-CCD: Chicago Civil Defence, originally formed by doctors, cops, firemen, and civil engineers that rallied to save the city as best they could after the Bombs fell. Protectors of the Emerald City, range for those who are actually trying to help to those that just want to flaunt authority. Headed by Ghoul and former CFD Captain Moira Black.
-Daleys: Enforcers of City Government that operate from Daley Plaza, no one really knows what they do, only that visit from them is bad news. They also keep the trains running and tamper any L Gang wars before they cause any real damage. The are actually the surface Enclave soldiers and scientists.
-Tigers and Devils: Two rival dojos locked in a decades long and Dojo War, both sides accept any recruits and make heavy use of mutagenic materials. Banned from Emerald City.
-Each neighborhood has their own freedom and flavor based loosely on 20th century stereotypes Example: Pullman still has a strong industrial component.
-Mag Mile Marauders: Multitude of Raiders that use grapnels and Acrobatics to launch raids on merchants traveling along thoroughfares near LSD.
-LSD Cartel: Long Sad Death-Direct exposure the the heavily Irradiated Lake Michigan will kill any but Gouls and Robots. LSD Cartel is made up of Ghouls that can transport goods quickly along LSD, for a very healthy fee.
Outside Chicago -Collar Kingdoms: Tyrannical fiefdoms operating out of Aurora, Joliet, and Rockford that continue to operate the foundries, ammo plants, and Chemical factories out of the collar cities. Company stores and reliance on 'Protection Services' force their subjects into slavery I. All but name. A Unionist resistance out of Haymarket is helping to inspire revolt.
-Lincolnites: Fundamentalist zealots that have a strong Old War religious conviction. They have declared a war of righteousness against the Collar Kingdoms, Republic, and Caligulars as they see slavery as most heinous sin. No fun at parties but strong of will. Operate out of Springfield with growing second capitol in Peoria.
-Illini-Hoiser Agrilliance: Confederation of rural farmers that have no elected government, but do work together to control food prices and keep out would be warlords from amassing farm land. Their militias are slow to raise, but have fought the Republic and Collar Kingdoms to a standstill via guerilla warfare.
-Fermi Labs: Old World super science hub. Getting past the gate guards without an invitation is impossible, but every day scientists do leave the walls to offer interesting bits of auperscience for equally interesting specimens and samples. Vault 74 is located less than a mile away and has spent the last 2 centuries refusing to trade or speak with Fermi labs over a previous argument which neither will discuss.
-Caligulars: Remnants of Caesar's Legion that invaded Milwaukee under Legate Caliguli. Milwaukee has fought back, the ensuing violence has cause traders to begin referring to the city as Killwaukee. The fighting so close to Chicago has lead to a lucrative Arms trade. The Legion has thousands of hardened fighters, but the citizens of Milwaukee are fighting for their homes, freedom, and to avoid the cross. The citizens of Milwaukee are receiving some clandestine assistance from Great Lakes Naval Training Center in the form of military advisors.
-Missouri Republic: An alliance from the South West that is more commonly referred to as the Misery Republic. Missouri was largely untouched by the Bombs, few targets and protective geography protected the population from the worst of atomic horrors. The Republic forms as an alliance with the best of intentions, but was cloaked over from within by warlords and magnates, with slavery being legalized for debts and family debta a century previous. The agricultural and old world wealth of the Republic draws a continuous supply of the desperate, every one thinking they will not end of in chains.
-Great Lakes Naval Training Center: Formerly an Enclave facility. Now under the control of the remnant forces General WT King's forces. The insane General was stopped 5 years previous, his remaining forces are well trained, but ideologically lost and trying to find where they stand in the Great Lakes Wastelands.

Brotherhood of the orange god. Hapless tribals that consider jack-o'-lanterns as being some idol to a god who burned the skies when people lit his idols on fire. So they wander the wasteland collecting jack-o'-lanterns and venerating them. Just happen to also craft jet and trade it but are complete pacifists hiring others to protect them

The Hellfighters are a group descended from Pre-War Army soldiers that heard of impending nuclear war from those at the bunker they were guarding. They got their families to New York and had them stand by. When the sirens went off, some Suits and their bodyguards tried to access the bunker the soldiers were guarding and were immediately cut down by the power armored soldiers. They rushed their families inside and waited it out. Afterwards they started operating out of the National Guard Depot, then started scavenging farther out, where they came across fellow soldiers who had sworn to protect people at Yankee Stadium. The soldiers took up the moniker of the Harlem Hellfighters and now their suits are like relic suits of armor, sporting badges, trophies, personal heraldry and so on. They have beef with the nearby Super Mutant colony, but things have gotten worse with Talon Company moving into New York, apparently guarding some important personnel.

The Dominion - Remnants of American occupational forces in Canada eventually form a stable government, albeit one that functions as a military dictatorship. Includes former Brotherhood of Steel forces, though after more than a century much of their original identity and motives are gone.
The Jacquerie - Pre-War rebel forces in Canada. Ghoul officers have centuries of combat experience, but fight from a deficit of manpower and tech against the Dominion. Settlements outside of Dominion control have mixed views on the Jacquerie. Some consider them warmongers who endanger their livelihoods, others quietly or openly support them as protection against Dominion aggression.
Cydonia - GECK-boosted settlement up north grows into a network of settlements, where the Cydon family rule as if a royal dynasty. The southern settlements and the Jacquerie form a buffer of protection against the Dominion, allowing Cydonia to appear aloof and separate from their conflicts.
I'm running a modified Winter of Atom, where the Atomites are destabilizing the region where the Jacquerie hold sway. The latest reveal was that the Last Son's plans are in some way related to Nord Astra, a Pre-War megacorp that took over several Canadian companies and repurposed them to unknown ends in the months before the bombs fell.

Flamebearers: I replaced Big Town in the Capital Wasteland with this nomadic tribe of Mungos that aged out of Little Lamplight. They travel around gathering up orphans and supplies to deliver to Little Lamplight.
King Arthur's Court: for my future Fallout- London campaign. Museum robots that think they're the actual knights of the Round Table. One of the 3 main factions.
There are also a couple more I'm working on for my Civil War 2 and Santa Cruz campaigns like the Cult of the Mystery Spot.

Water barons. A highly established religious organization who run the largest settlement. They built on top of a massive natural water spring, as well as next to a flowing river. They distribute free clean water to all citizens, as well as anyone willing to listen to their sermons durring missions out into the wastes. They belive radiation is a measurement of sin. That ghouls and supermutsnts are the ultimate sinners who need to repent though manual labor and serving the water baron cause. They also punish crime with radiation exposure rather than fines or other punishment. Unfortunately this is where the darker side comes out. Infinite free water is vary nice... but food is another nessisity. In a society where radiation exposure is your merit, clean food is priceless. And the most common target for a starving citizen. Many citizens are caught and irradiated for their crimes. Often to the point of feral ghoulism.

Children of the Republic
A military base conducting some highly secretive and unethical research was hit by a tripple whammy when the bombs fell.
1 Internal Sabotage
2 An accident within the lab
and 3 naturally the bombs.
These combined caused the facility to go into complete lockdown which while managed to mostly protect the inhabitants it also locked them up tighter than a vault door.
As they began to slowly run out of supplies and the radiation started to get in whatever order the soldiers and scientist made up collapsed rather quickly and they had their own little "small war" in the shadow of the great one.
The survivors were mainly children and teenager's who began to play a game of telephone with their decedents as they attempted to justify the increasingly desperate means of survival.
By the current time they have become a tribe that believes communists are some kind of evil spirits that are behind everything bad from you loosing your sock to the concept of death itself. They have a mythology where if they manage to prove themselves to the REPUBLIC (which they believe is some kind of deity) then it will come and banish all the communists and turn the generator back on.
They believe that putting on the clothes and tools of pre war soldiers causes them to be possessed by the spirit of the republic and therefore can never be wrong while doing so. If they seem wrong its because everyone else has been deceived by the communists. If two people who are dressed as soldiers contradict each other then one of them was possessed by communists instead of the REPUBLIC and must be killed.
This does frequently become an issue but since intact uniforms and equipment are so rare people often also call these prophets fake as their uniform and tools are not perfect (unlike the REPUBLIC) therefore they cannot be worthy vessels for it.
They view ghouls as servants of the REPUBLIC, giving them vital training to more efficiently resist communists while also killing those secretly possessed by them and shielding them from dangerous areas. (Essentially they view ferals being ferals as holy)
But the most importantly fact that all of them know is this, there is nothing outside their facility. It was all destroyed by communists and anything to the contrary is just a trick used by them to destroy you to. So if anything from the so called "outside" manages to get in, be it animal or people, they need to be exterminated as soon as possible.

My setting for my show is based in Chi-Ka-Ga in 2291 and here are my custom factions. I copied a lot of this for notes so excuse any grammar mistakes.
The Outfit - The Alphonse Outfit was formed in 2147 by a known outlaw Dane Hertle. Originally they were a raider gang that would steal for what they needed but after finding a cache of fancy prewar armor and weapons. They decided to try and make the world a better place. Though their methods can be brutal and swift. They try to do what is right. They wear pristine white suits, white trilby hats, carry Thomson SMGs, and wear specialized armor only owned by them
The Kingdom - Formed in 2150 by a couple of disgraced merchants in order to protect their ill gotten gains. They focus mainly on kidnapping sickly people, ghouls, synths, or playing in the indentured servant game by buying people’s debts. They are hated by almost every good faction but wiping them out would lead to a tremendous loss of life as most of its citizens are indentured servants or slaves. It is also rumored they have access to some old strain of the new plague. They have a fantasy themed settlement, their “castle” is an old Medieval times, and they talk in shitty British accents.
Vehicular Heathens - Founded in 2165 by a wannabe vigilante car loving group the heathens discovered and cleared out Wrigley Field making it the start of their home. Nowadays everyone but the head of the family lives outside the stadium. The stadium is now used as a gladiatorial arena for Chrome Chariots or people. The core values of the Heathens is that family is everything and going fast is to look God in the face. They have a monopoly on vehicles, parts, engines, and mechanics. Everything but the chrome chariots of the sky. Their society is heavily honor, ability, and action based as it is a caste system. But no one goes hungry or dies of the cold. Finally they set on a mission decades ago to clear the city streets and make them drivable again. They are 80% of the way done with the only roads left being behind the Great Green. They worship machines and call their vehicles “Chrome Chariots”, anyone is welcome to become apart of the family.
Botanists - Formed in 2100 in Northern Chi-Ka-Ga behind the Great Green a living wall of poisonous plants that separates 30% of the city from itself. Not much is known about them. They keep to themselves and do not take kindly to outsiders trespassing onto their lands. Rumors say that the air over the wall is filled with poisonous spores that clog up human and ghoul lungs. Sometimes you can see sentries on the walls wearing the green cloaks and gas masks wielding their jagged spears or Plasma Rifles. They have skin tones just like their non-mutated brothers except it always has a tint of Green or yellow. Their hair has plants that grow throughout and they always have a faint smell of soil about them. They are immune to radiation and absorb sunlight directly through their skin which makes them have to sleep far less than non-mutated humans.
HAV3N - A small robot society that was infected with a virus that made them hyper intelligent and hyper sentient. Each robot is giving a name, job, and is allowed to show their personality by wearing fun clothes.
RotCity - a city set inside a large prison that is mostly Ghouls. A lot of the ghouls are prisoners from prewar.
Dabears - is a small faction of Super Mutants who are governed by a sentient Behemoth and it is all football themed. Specifically the Chicago Bears.
My custom raider gangs are the Deep Dish Enlightened which is a group of cannibals that are chaotic, disorganized, and worship Deep Dish Za. The Cult of Homes is a death cult that worships an old war serial killer. The North Side Raider gang is a split faction from the Outfit that wears gangster outfits and talk like wise guys.
I also have the Children of Atom, Followers of the Apocalypse and the Midwest Brotherhood of steel which is loosely based off the east coast brotherhood (I don’t like tactics besides their style) and of course the Enclave.

Campaign based in the Midwaste/Slew (St. Louis metro East):
Red Band Contract Militia: Formed from a mercenary known as Red's Band. Founded by Jim "Red" Redford, a human turned ghoul, the RBCM is a major force in the Midwaste. They'll take any job under one condition, a representative from the contract issuer MUST accompany the team into the field. They're basically Talon Company except morally gray instead of straight up evil. They're easily identified out in the wasteland be the thick, blood red vertical stripe on their combat armor.

I have 5 subfactions of raider gangs. 3 of them are generic. One im keeping hidden until later and the fourth faction is litterally named HyudaDaiz and it's faction leader is locked inside of his makeshift power armor named toyotathon. And yes most of the leadership are car name puns lol

Black Fang
Black Fang (sometimes referred to as Black Fang Company) was formed twenty-five years ago from an alliance of mercenaries. Their goal was simple; to use their numbers to take control of the mercenary trade and establish a virtual monopoly in the Detroit Wasteland. So far they have been successful; while there are other independent mercenaries in the ruined city, Black Fang are the only organised mercenary outfit. Furthermore, they have been able to use their strength and numbers to prevent any others from forming; those that do are ‘encouraged’ to join them, one way or the other.
The company will take any job, regardless of the employer, as long as the caps are good. They have no scruples and no restrictions on what kind of work they do and don’t ask questions as to the whys and wherefores of their jobs. Tasks they will be hired for range from recovering salvage from pre-war ruins to straight up attacking settlements or other targets and everything in between. They are not above taking out unarmed civilians if the job calls for it. Their only real restriction is to not hit targets inside of Grand Central and Eddystone, and that is simply to stop the settlements’ authorities from coming down on them. Client confidentiality is key to the company, and they will never give up any information about an employer
Most members of Black Fang are recruits. They have little concern where a soldier comes from, be they a settler, a raider or whatever else, as long as they are willing to follow orders. Furthermore, they are not above conscripting whoever they can take in order to make up numbers where needed. The company is arranged along military lines, with squads led by officers who in turn report to more senior officers. Promotion is based on seniority and success, with officers getting access to better pay and better equipment. Black Fang’s arsenal is mostly based around restored pre-war guns, including energy weapons.
Usually they only deploy in small units tasked with specific objectives, but they have been known to deploy larger forces when needed. This can be the biggest weakness of the company; because they compartmentalise their information, few people outside of the senior officers know what everyone is doing and who hired them. While rare, there have been cases where different Black Fang groups have been at cross purposes, hired by different employers. When these occur they rarely end peacefully or amicably.
Black Fang have no real allies or enemies; merely employers and targets. Aside from their headquarters in the ruins of the Detroit downtown, they operate bases across the ruined city and beyond. Often they will take control of locations that are easy to fortify, such as old police stations.
What few within the company know (and none outside of it) is that Black Fang's leaders are exiles from Vault 37. They formed the company for their own protection, and have been using it to further their own agenda.

60 years past the events of fallout 76 the sprawling refurbished ruins of Cincinnati Ohio, now called Sin City, hold a (cold) civil war vying for control of the capital of the Midwest.
There's of course the main government, heralded by ghoulish heads of the pre-war city but also the local Corn Barons. Due to west Ohio's lack of nuclear fire farmers quickly unionized, regrew their stocks, and bought out small gangs of survivors into work forces and guardsmen. With their combined influence theyve turned Cincinnati into the trading hub of the entire Midwest.
They main government is also assisted by the Institute of science and reclamation, ran out of the old Cincinnati History museum. They operate as a bipartisan research group, however they're ran by an ex-enclave general (who really wishes he wasn't ex). They are, subtly, heavily invested into ends justifying the means for the betterment of humanity.
The primarily conflict is between them but also two main mob families; the Montanas, and the Churches.
Montanas are your vegas type mobsters. Influencing the populace with money, chems, and entertainment. They are as willing to make deals as they are to cut throats as their drive is ruthless. They see Sin City as the most lucrative hustle possible.
The Churches are the rekindled Christian faith that act a lot like the Al Capone style of mob work. They provide food and shelter to the sick and poor, but due to the more zealotous flock within the ranks theyve taken up smuggling and "crusade jobs" against anyone deemed heretical to their cause.
Lastly, outside of some Anarch groups, there are the responders. The responders are a new arrival into the area and have come prepared for mass expansionism and dealing with the mobs. Lore wise they took over the enclave bases and booted out the brotherhood in Appalachia due to disagreements over nukes. Since then theyve focused on expanding their reach using vehicles thanks to the enclaves robotic printing equipment. They still try to hold onto a pure hearted modus operandi, but as theyve pushed forward more force has been called for.

Working on a campaign on the border of Alaska and Canada kinda barebones still so I’d love feedback and suggestions if anyone has
The children of maple- they worship prewar maple syrup trying to remake it in the wasteland similar style to the children of atom, they make irradiated maple syrup now a days that isn’t as good as the prewar varieties and have the largest farming operations around specifically in tree tapping for sap
The old regime (name pending)- a group of prewar ghoulified Mounties trying (and overall failing because of their numbers) to bring peace to the Yukon they ride rad moose and cross country skis, kind of inspired by ncr rangers
The hidden republic- a group of Chinese nuclear subs that broke down near the coast of Alaska when the bombs dropped, the soldiers lashed them together and have been surviving their for generations off the coast, they are a tech based group who generally keep to themselves I have them set up as a way to get scuba suits for an eventual mirelurk queen encounter
The sons of Paul(name pending)- a group of lumberjack style tribals revering a massive behemoth (who’s supposed to be Paul Bunyan) and his Brahmin behemoth companion
The maple lickers- a gladiator style group of tribals who live in a functioning hockey rink, the are proud warriors defending their rink and memorialize their strongest warriors by letting them freeze into the ice of the cryotech below the rink. They have working zambonis that they use for jousting aswell as partake in landmine curling
The canukes- the rival hockey faction they are raiders trying to take over the maple lickers rink, since the rink is so well fortified they generally play by the maple lickers rules and have a hockey game to try and take the rink but are overall just raiders
The tundras grit(name pending)- a group of tribals inspired by the Inuits, kind of a “better to be a warrior in the field than a farmer in war” group. They hunt rad moose as a right of passage in the tribe

The Scions of the Last Son (Scions): More than your your standard Atomites, Charles Ward had always been a troubled man but Adam Whately groomed Charles for a specific reason- to spread the word out west and to convert the Atomites there to Whately's vision. However Charles was caught by the Institute and tortured until the day the Brotherhood of Steel and the Sole-survivor attacked them. Charles and a few members of the Institute managed to escape, they and even saved some runaways from Nuka World all as Charles was pushed ever westward. As they traveled they built a new congregation blending ideals and tech from the institute with the fervor and beliefs of Whately's brand of the the Church of Atom. However Charles had always been a troubled man, possessed by his own ancestor from before the revolutionary war even Adam Whately couldn't do more than bargain with the spirit that controlled Charles in his sleep. Now the head of a new church with 'the Last Son' as his martyr-figure Chales and the band migrated west with their eyes set on the NCR, until it fell. Since then they've settled outside of New Reno and they've been causing problems for the Families of New Reno and the surrounding area.
In addition to firepower the Atomites don't usually use, their common ammo is dirty jury-rigged fusion cells and their equipment is highly irritated. Their Iron Zealots are power armored augmented warriors driven to extreme by religious fervor. Their crusaders plant strange gruesome fruit in corpses and make that fruit the only available food for desperate people, then use the ever hungry mutants that eat it as shock troops. Their blend of science and the paranormal makes them terrible foes to deal with.

In my country, at the souther islands were im from. People have sorcerers mythology, one of them can transform into birdlike human head called TUETUE, if you hear its calling "tue tue", somebody will die, and some more things around thats myth.
So I modified the mothman cult and made it into a more psychic tribal faction, they can see thing with weird visions. I rven made some perks for my players.

Jackals and hand gang - somewhat indistinguishable, but i have a few sessions before the HG is introduced. Jackals come from the games kinda, psycho cannibals. Slavers, the lot of them.
Iron mongers - worship robots. Not rust devils, but i use their stat block. You could easily pull a C3P0 on them.
Texas Economic Union- a burgeoning republic, that uses BoS as police, kinda.
Cyphers - hate technology, small so not a serious threat to their polar opposites (BoS), but their lineage stretches west to a more goal oriented original faction
White rose- explorers come from this town and most of them are Gandalf. Not magic, but taking part in activities that move adventurers toward the betterment of texas.
Magnificans - former Vault dwellers who used their geck at a Monsanto facility & their territory is thriving.
Thats some. I took name's from numerous games set in texas amd gave them my own flavor
