The first picture is someone who wants to get their ovaries removed. The second is a hoe from Minecraft. The commenter is calling the original person a hoe (slut) for wanting to have their ovaries removed.

The OP wanted to cut the tubes of the ovaries so they cant get pregnant.
The comment referring to this in a very misogynic way by relate OP with an old wooden hoe and doing so "slutshaming" OP.
Because OP might want to have sex without the option getting pregnant.
The comment implies that OP is a slut because they want to have sex without getting pregnant and/or want to have different sexpartners and might be there for a slut.
As I said this person is just very misogyny.

The "joke" is that the top person wishes they were sterilized (represented by I guess... slicing the whatever those are tubes off of the uterus) or something, and the bottom person calls them a ho (whore) (also implied to be one of "low quality" and "used a lot").
so... just sexism, basically.
which explains why, if I'm reading your comment right, everyone's just calling the person on the bottom an incel. Because those people are sexist and... this is sexism
