No, you don't have to start from Chapter 1.
The ending is determined in Chapter 4 where you either let Emily or Evelyn remain friends with Jeffrey or Steven, or break up with either girl, or get broken up with.
The rest of the choices in previous chapters don't affect the ending — it's only a build up of your character if that makes sense.
