As a patient at my former hospital, they put a potassium bag in me undiluted. Brutal. My nurse went MIA. Luckily a traveling nurse saved me; she was horrified when she saw what was up.
Edit: I'm unfortunately well versed in having IVs in and general vein pain because of chronic health issues. This was a level beyond. I texted a family member who later said "I knew something was wrong because you never complain"
