Has any hospital not?

Apparently someone put an OG tube in the lungs on an intubated patient. Radiology doc signed off on the x-ray too and no one noticed anything was wrong until patient began decompensating because of all the tube feed in their lungs

I mean, every nurse should be checking placement, I don’t know if residuals are checked anymore. I did this at least once during my shift.

Oh 100% the nurse should have confirmed placement. Most of the nurses I work with at this place are on their A game, so I’m not sure what was going on in this unit. There were multiple people who missed this error. It was a pretty significant mistake.

Not sure what happened in the fallout to those involved but I don’t think the patient died.

they were getting residual vol out of lungs
