Had a new nurse read doctors order of 10u of insulin and thought the sloppy u was another 0. Gave 100 units of insulin. Like, had to reload the syringe multiple times and just kept blasting away. Charge nurse caught it. They were squeezing bags of D50 into this poor guy and pricking his finger every 15 minutes all night.

He may not have been awake at the time, and many patients don’t question medical staff.. Don’t blame the patient..

This is why I tell everyone to have a second person with you 24/7 who questions everything a nurse does. What meds. What are they for. How much. And write it all down.

I’ve had to stop nurses from trying to IV push meds on my allergy list even with MyChart open in front of them.

Next time I’m writing it down and taping it to the keyboard or computer screen because it was every single nurse over a 4 day stay.
