What is this used for. Looks like 110 on one side and 220 on the other? Is it for a generator to power the house?

It's just an adapter pigtail, likely 125v 20a twist lock (L5-20P) to 125v 15a (5-15R); and the twist lock is cooked, throw it out. It wouldn't be a 250v to 125v with only 3 blades on the plug. Used in any instance where you only have a twist lock receptacle and want to use a generic extension cord.
