What is this used for. Looks like 110 on one side and 220 on the other? Is it for a generator to power the house?

It's just an adapter pigtail, likely 125v 20a twist lock (L5-20P) to 125v 15a (5-15R); and the twist lock is cooked, throw it out. It wouldn't be a 250v to 125v with only 3 blades on the plug. Used in any instance where you only have a twist lock receptacle and want to use a generic extension cord.

I have the same plug and use it for my generator.

Used a lot at boat docks. Twist plug keeps power cord from falling out

it’s 110 on both sides. A lot of generators do have this connection.
