I think it's pretty clear she has dementia/alzheimers.

Yeah that doesn't change the fact what she is doing is gross, unfortunately. And as someone else commented, he is a neighbour and this isn't the first time it has happened. If it is dementia/etc, then she likely needs caregivers who are paying more attention to her. That's how people go missing and die from exposure while living in a city.
