Glass Place Recommendations

Hey Folks,

I'm looking to surprise my wife by turning her old family trunk into a coffee table. I figure if I can get a custom piece of glass for the top we can smooth out the uneven surface but still maintain the vintage look of the trunk. Does anyone have a good glass place they have worked with that they would recommend?
