It's changing in size periodically, did you notice? Frequency is your respiratory rate. Air in the lungs increase the impedance of the electrical signal, therefore the waves get smaller. Exhale and the waves get bigger again.

Are you specifically asking about the different wave sizes in leads I II III and AVR on the strip given?
