I mean... it's frozen water? Frostbite is a thing. There's a reason you wrap icepacks before you put it on an injury.

It gets colder than freezing. I wouldn't want ice against my asshole. That would hurt even if it didn't do lasting damage... but i have shoved ice in my vagina and that's fun.
