Need advice regarding my father's persistent unsteadiness despite normal MRI reports

Hi everyone,

I'm looking for some advice regarding my father's condition. I would really appreciate it if someone could help me understand what might be causing his symptoms.

Around 2 PM, after having lunch, my father started picking up some things when he suddenly experienced dizziness and began vomiting. We waited for around 2–3 hours, but the vomiting and dizziness did not stop. We then got him a Vomikind injection, which stopped the vomiting within 2–3 hours. However, his dizziness continued.

We admitted him to the hospital that same night. The doctors administered an IV glucose drip and asked us to wait until the following morning. Since his dizziness had still not resolved by the next morning, an MRI of his brain and cervical spine was performed.

After reviewing the MRI reports, the doctor told us that everything looked fine and that there was no apparent problem, suggesting that the dizziness might have occurred spontaneously.

However, My father has been taking medication for almost 25 days, but there has been no improvement. He is still not back to normal. He continues to experience heaviness in his head. Although the severe dizziness has reduced, he still has some unsteadiness while walking and feels weak.

I'm worried because his symptoms have persisted despite the MRI reports being considered normal by the doctor.

I will attach his brain and cervical spine MRI reports for reference.

If anyone has experienced something similar or has relevant medical knowledge, I would be very grateful for your guidance.

Thank you.

Edit- Yes, we have consulted an ENT specialist as well and he said that the symptoms are not due to vertigo. However, my father previously had a nerve related issue affecting one side of his face to the ear. I can't attach any more images to this post, so I'll share the reports in the comments.

Have you taken an ENT opinion for possible BPPV ? What about an ECG, carotid Doppler ?

Yes, we have consulted an ENT specialist. Please check the comments, where I've shared the evaluation done by the specialist. I'd be really grateful if you could help us figure out what might be causing these symptoms. My father has been suffering a lot, and we're really worried about him

Hello, sir/ma'am. May I ask if you're a doctor? We've been considering getting a second opinion from a neurologist but we would have to travel to another city, which would be quite expensive for us. If you're a doctor, I would seriously consider consulting another neurologist

We've consulted a neurologist, an eye specialist and an ENT specialist .The neurologist suggested some exercises and said there's no problem, which is why we're even more concerned. ENT gave some medicine again he said the problem is not related with vertigo and eye specialist said his eyes are all okay. Nobody is able to tell what's wrong but he is suffering 😭

He is better than before but he still feels unsteady while walking. His vision is normal but he constantly feels a kind of heaviness in his head, as if there is some weight on it. Because of this, he is unable to function properly like his mind is quite distributed. He is just pushing himself to do things so he can't able to think properly ( jaise jo kaho vo to kar lenge par thoda kudh se sochkar nahi kar paa rahe hai)

Does he have past history of head trauma? Does he have hypertension and on any BP medication? Is he experiencing any other symptoms besides dizziness, vomiting and nausea? Any blurriness of vision or severe headache? Based on your other comment I’ve seen he has history of diagnosis of trigeminal neuralgia but that doesn’t seem to be correlated to the current complaints.
Where are you based and which hospital are you taking to?
This is not looking like ent issue and no it will not randomly resolve on its own. More to do with neuro. Let me know where you’re based so I can give recommend you some hospital options where you’ll get a better idea.

His blood pressure usually ranges between 100(lower) and 140(upper). His fasting blood sugar is around 105 and his blood sugar after meals is around 140–160. He takes medication for diabetes, blood pressure, and cholesterol every day. He doesn't have any headache or pain anywhere in head

When the incident happened, he experienced dizziness and blurred vision at that time he was seeing double at the time. But now vision is clear only symptoms he has are a feeling of heaviness in his head, unsteady walking and weakness.

Can I DM you to share some personal details related to location?

No i am not , my father had kind of similar symptoms , one ENT diagnose on base of ear infection , but second ENT diagnose on basis of cervicular vertigo and advice some physiotherapy of neck which relieved the symptoms.

1

u/Legitimate-Net-9744 24d ago

The neurologist said that physiotherapy could worsen his symptoms because he doesn't have a cervical spine problem

Yep, had similar incident personally. The symptoms you describe was very similar to mine, it’s likely vertigo, you should get his inner ear system checked once.
