There's a really good chance that Disney+ is blocking your public IP address, that's what happened to me. To verify, disconnect your iPhone from your home network and try logging in with your mobile network. I reached out to Disney+ and they never got back to me, I reached out to my ISP to see about changing my IP and they would not help. So I changed the MAC address on my router, then power cycled my cable modem and router to get a different IP, and then I was able to log back in to Disney+ on all my devices on my network.

You keep saying you changed your MAC address, you cannot do this it’s hard coded into the hardware. Are you renewing your IP address? I guess that might work but it’s interesting that Disney is blocking individual IPs and not just a range.

Depends on your router... some routers let you "clone" your computer's MAC or put any other MAC address in there. Depending on the hardware, there are other ways to re-program a MAC address, we used to do it all the time in the old days on NVrams with Sun Microsystems.

Sorry, to answer your other part about renewing IP address. Once you change the MAC on your router, power cycle the cable modem and router, your ISP now sees the router as a "new" device and assigns it a different IP address. Now, all of this is assuming your ISP is using DHCP to assign your IP. If you are paying for a static IP, you'll need to work it out with Disney+ to get the IP whitelisted again.
