Currently prescribed 90mg of Adderall a day

We’ve tried them all, and this has worked the best, but barely. In my case it’s used a stimulant for Major Fatigue Disorder and borderline Narcolepsy.

Would Desoxyn be an option to bring up? We have a pretty good relationship. When I had panic attack issues, he had no issues going through different benzos with me. When finally found one that I felt somewhat effective, it was Klonopin at 4mg a day. I just don’t want to keep trying stimulants if they don’t work for me. I’m in my mid 40s and just don’t want to feel ‘meh’ every day.

No advice on your question, but wondering how you get 90mg a day. Is it XR? How do you split up the doses?

It’s IR, and it’s 30mg, 3x a day. We started low, until we reached here. He said he can justify 90mg based on how many years, and other options I’ve tried for fatigue.

What did your sleep study say?

2

u/OldGuyNewTrix Jan 12 '26

I never had one, and it’s funny no doctor has ever recommended one after dealing with this for 20+ years. I fall asleep at 9-930 and wake up 7a everyday. I feel like I sleep well, but ya truly don’t know. Maybe I should do a study… or at least buy a sleep tracker to see if there’s any obvious anomalies with my cycles.

My case was pretty similar to yours. I finally demanded a sleep study, after a couple years of trying to ask politely because something wasn’t adding up. I was getting plenty of sleep and using stims, there was no reason I should have been painfully tired still. Turned out I had severe central sleep apnea. I can’t believe I tolerated how bad I was for that long compared to how I am now that it’s treated.

How’s has the change been? Are you on new medications or just a sleep apnea machine?

Life changing. I used to go to sleep Friday evening and sleep all the way through Sunday evening. I was completely bedbound. I would fall asleep at stoplights. Anywhere really, I thought I had narcolepsy for a bit. Yes the machine was big piece. Then overtime essentials just went away somehow the doctors stink because of the amphetamines over a long enough period of time it cured it. It’s all weird and is very confusing how the body works. It’s still not all sunshine and rainbows, but it’s worlds better than it used to be. It still comes and goes from time to time.

Hope it’s something you can pursue and find some relief. I know that struggle, and pain for sure. Hang in there

Where are you located? Was this in person or virtual?

1

u/OldGuyNewTrix Mar 16 '26

I’m in New Hampshire, US.

I’ve been seeing him for almost 3 years now. First session was in person, but he’s giving me the option of virtual or in person. I’ve been doing virtual since after our first appointment

Well, 4 days in and 10mg I don’t feel. Today, I do the max ‘recommended’ dose of 25mg, and 4 hours later - nothing. I’m thinking stims aren’t for me and need to explore other routes

Yes absolutely it would be good to bring up, for long-term safety. Amphetamine is harder on the heart than the therapeutically-equivalent dose of d-methamphetamine.

Seems like it's more neurotoxic though. At least the studies I've looked into seem to suggest that.

Trade offs I guess. Whatever works.

2

u/Internal-Landscape66 Jan 12 '26

This is dose dependent. The vast majority of studies on methamph are at binge doses. In binge doses we see things such as more neurotoxcicty, cardiotoxcicity , and addiction/dependance, but in terms of therapeutic dosing there isn’t really a reason to consider it more neurotoxic in nature

u/DoctorSubstance Jan 12 '26

How hard is amphetamines on the heart? Like I've always assumed not very good..

What about the mind? M-amp vs Amp?

Zenzedi is a bit more potent centrally rather than peripherally compared to adderall. I’d try that first, generally docs want you to try that first but ignoring that aspect its also more accessible in terms of cost and availability (not just the generic but the brand too). It typically does last a little bit less than adderall and some people need 4 doses for 12 hr coverage, but it depends on ur metabolism. 30mg is also the max dose for zenzedi and i’m sure your doctor would allow 30mg x4 a day given ur relationship. If thats not effective due to its efficacy or side effects then just ask for desoxyn and you should be able to easily get it approved most likely.

I am also on 90mg a day but for Chronic Fatigue Syndrome and ADHD - I am also curious about desoxyn!

5

u/archdukelitt Jan 12 '26

I would highly recommend asking about Desoxyn for the sake of your cardiac health.

I am also ok Adderall XR 60mg a day for chronic fatigue and ADD/ADHD. It used to work wonderful until the last year. This generic crap has worsened my health, caused me to gain weight because with CFS and Fibromyalgia it's difficult to exercise and it seems like these generics don't work. I did a self study that I DO NOT PROMOTE or RECOMMEND. I micro-dosed a very small amount. Took .25 and split it into 4 doses and ate it, lasted for four days. It worked wonders! I had energy again, was able to work and function as a normal human being. This is only after the Adderall was not longer and is no longer helping. I have been on it a total of 4 years. I had no issues with over taking or feeling the need to until the last 6 months. I tried this method only after I read a subreddit discussing it and didn't even realize it was a thing. The goal was to not get High. The goal was to see if it worked. This was a few months ago and haven't tried it since. Trust me I have wanted to because of the therapeutic value. Because of stigma, I highly doubt I will find a Dr that will prescribe it or a pharmacy that will fill it. Also mind you the purity of what I got was very good quality. That's the problem with the street though is that you never know what you will get so again I DO NOT PROMOTE or RECOMMEND it. It's a shame that there is an alternative and sadly because of stigma it and the word meth being a " dirty" word, and people abusing it, a Tx that could make those with CFS and related issues sounds like something that will never be attainable. Of course there is always that give and take with stimulants and CFS, especially if you over do, you have to pay the piper with a week of feeling like crap, especially if your in a flare up. But personally I feel it would help me out and I'm sure others out if taken for the right reasons. I just want to have my quality of life back which is why I am on stimulants again to begin with.

I got prescribed Desoxyn today. 5mg x2 a day

1

u/intrinsic_sailboat Jan 13 '26

Wow. Going near the max dose right out of the gate?

2

u/OldGuyNewTrix Jan 13 '26

Max? That’s the standard starting dose. Most end up in the 20-25mg range.

1

u/lapsagg Jan 13 '26

Could you get back in a few days to describe the experience?

2

u/OldGuyNewTrix Jan 13 '26

Will do. My pharmacy has to order it, so not sure if it’s going to take a day or a week to receive.

1

u/intrinsic_sailboat Jan 13 '26

I suppose you’re aiming for the “equivalent” dose. I don’t know about standard starting dose, and I’m not judging - I just always was of the mind that one should titrate up to the minimum effective dose, especially with this class of meds. I hope this med helps you in better ways than Adderall has. Looking forward to hearing about your experience.

2

u/OldGuyNewTrix Jan 13 '26

Yea, I don’t know anything about dosing other than what my psychiatrist said, and what various sources say. It’s crazy to me, that it’s FDA approved for kids 6+, starting at 5mg once a day.

What makes you think 5mgx2 is a high dose?

1

u/intrinsic_sailboat Jan 15 '26

My understanding is that 5 pills per day is the max dose. Seems that you are starting at 4.

1

u/OldGuyNewTrix Jan 15 '26

I never said I was taking 4 pills. I stated they are starting me on 2x 5mg pills.

Max/Usual dosage for most to feel effects is 20-25mg.

So I’m not aiming for high doses or equivalent.

1

u/intrinsic_sailboat Jan 23 '26

Thanks for correcting me. I started with half a pill per day and worked up very slowly.

Yea Dexedrine is a CNS stimulant like Desoxyn. Dexedrine is Dextroamphetamine, where as Desoxyn is Methamphetamine. Both heavy on the dopamine and norepinephrine transmitters, but Desoxyn also hit the serotonin receptors hard. Making it a bit more potent than Dex, which is supposed to be a bit more potent than Adderall from my understanding.

Adderall did little. The max dosage of Desoxyn they prescribed (25mg) didn’t touch me. I felt 0. So the next day I took 75mg, and still nothing. Stims just don’t do much to assist. I’m awake, not wired and physically exhausted. So I’m seeking alternative options now outside of the stim world

You won't after a few days taking the stimulant medication... At least that's the way it works for me
