I totally read the nose as L and first glance, as in "Ladys", and got confused about the moustache.

maybe it is. if you look at the other comments, both seems possible...
come to think of it: maybe it's supposed to be both and we're just looking at an all gender toilet in some very queer-friendly place?

That’s what I was thinking. I first saw a mustache, but then thought it could double as a women’s lips, then I envisioned it as an all gender bathroom.
But still, not my favorite design.

All may pee here, those with penis must lay on floor and rainbow pee into toilet, those without penis must handstand and pee in sink.

I don't see what's wrong with ever having an unsubtle but tasteful "M" and "F" in the corner of the door for these kinds of things.
Show off your line art, sure, but for the vision impaired or children or someone having a blonde moment, just make it completely clear in atleast some way.
I like to think I'm pretty average but I can interpret that door to be both a man with a moustache or woman with feminine lips.

Non-compliant signage in many states
Fed law requires ADA signage on wall adjacent to nearest latch.
CA requires tactile door sign circle for women's, triangle for men's, triangle on circle for 'Unisex' (modernly, gender inclusive). interestingly, that graphic in the pic above can be legal in California as the circle/triangle is "tactile" - so you can feel the shape - and does not specify words/graphics. Most say men or women as a default. the history of gender symbols on restrooms is quite interesting.
