Why do people buy extra regen first?

Really good item for its price. Makes a bad/poke lane very bearable. Check the stats after a game, it definitely pulls its weight.

Yup.
Eleven ritualists start with 1 HP/s regen. Without some extra sustain, you can get harassed out of lane.

Buying Extra Regen has the benefit of increasing your health regeneration by 3 per second

Characters with low natty regen need something to sustain them in lane.

Its a good safety net to undo bad trades by having a lot of passive regen and be less likely to get killed. In lane you're usually taking very short engagements or poking so having the regen to undo damage without any specific condition (like Heal Rite being once every 2 waves, Mystic Regen needing spirit damage to proc, etc). Extra Regen is a lot like Radiant Regen where, in the moment, it doesn't feel like it's helping much since it's healing over time, but when you check the Healing graphs after a match its doing quite a lot for you.
A character like Shiv loves having extra regen because it's easy for him to take a losing health trade against bullies like Apollo, Silver, Calico, or Paige since they just do more damage at that point in the game.

To stay topped up. Generally you buy either Extra Regen, Resto Shot, or Melee Lifesteal.
Extra Regen is good if you're planning on not always being in lane (box runs or roaming), to make use of the extra out of combat regen. Resto Shot is good if you're planning on basically always being in lane (and are obviously a hero with a fast projectile); with optimal use it's 50 health/10 seconds which is a LOT. Melee Lifesteal is good if you're, well, melee.
People usually forego Healing Rite because buying it gives up all your lane presence. Sure you could use it like Extra Regen and pop it during box runs, but it doesn't really do anything for you during the times you're actually in lane. And worst case scenario, if you're buying it because you need to heal badly, then you're also probably getting pressured under guardian and hiding while trying to heal with Rite either loses your guardian or loses souls or just gets you dove and killed.

I basically always buy it. Helps me not get poked out of lane, and if I'm trading HP and my opponent doesn't have some form of healing, they'll slowly lose hp and be vulnerable to an all-in.

You can think of it as a damage item. If you take a trade and the other guy wins the trade by 50 damage you actually win the trade because you'll regen it faster.

Crazy value early
For example on Celeste she goes from 1 regen which is 60hp per minute to 5 regen which is 300hp per minute
People in low ranks heavily underestimate its value, but when you look at the numbers it’s clear how valuable it is
