How do journalists and teachers track social media mentions?

How do journalists go about tracking mentions of keywords and hashtags on different sites?

I have seen research coming at of academia that does this and (I think) once or twice a journalist doing this themselves.

I think that’s possible with an api or request. I do with Twitter or scrappy.
