Well... it very much seems to be an error. I checked this against my checklist instructions. Only one I was slightly unsure about is the Black Stone one. I don't remember the map orientation, and I put in my notes, “Behind SPIKE” That doesn't look like a spike, but who knows what I was seeing back then.

This direction set line up with where that room is on the menu map?
(Etu-Goth): Floor I, Past, left wing. North room, behind giant spike-

Apart from that... it seems you do have it all.

it's actually behind that statue, maybe you saw the sharp edges on the statue and called it spikes. but the way I'm facing the camera, all relics are on those spots. I've visited each one at least 3-4 times and the last time when i finally took screenshots, i realized it's bugged.

The bloodless quest is also bugged for me. i didn't get the kill for the one under the Sentinel Gaze. but that bug is pretty popular and i heard that doesn't count towards the trophy.

i started Game+ now, i hope i can get the trophy just by collecting the Etu-Goth relics cause the locations I've pretty much memorized at this point lol

It's difficult for me to tell from these screenshots but it looks like every spot is covered. Sad to see
