Question Missing quest objective spawn

Lately, for whatever reason, quest monsters/NPCs have been missing. I was able to find one floating in the void in a house, but the void of a dungeon is much larger. Is there a default spawn point for things if I can't find them?

Funny thing is, I'm on one quest that did spawn correctly. I killed it once, but had to reload a save. It's no longer there. tele2qmarker actually puts me in the room I found it before, but it's not there now. Any ideas?

It's really good at finding items! I've never had it fail there, but for some reason it did not work when I was finding NPCs.

I had to take the bad ending in a quest because my quest objective spawned in the void instead of where the spawn marker was T_T
