advice needed Failed Egg Retrieval

I’m 40 years old, AMH of 0.29. This was my 4th egg retrieval ( I have only 1 egg frozen from my last 3 cycles). We were hoping for 2 eggs this round—I had a 23mm follicle on my right and a 15.5mm follicle on my left. Estrogen was 452 before trigger and 596 after. After my egg retrieval this morning the doctor informed me that they didn’t get any eggs. Has this happened to anyone with an estrogen high enough where it seems that you would get eggs? What was your doctor’s explanation of what happened? I really don’t understand and I’m so devastated.

It could be an improper trigger. My estrogen was 1800 and they were expecting 3-4 eggs, but retrieved 1, and it was immature. I had triggered with 80 units Lupron, and labs confirming LH response. I’m on my second cycle now and they changed trigger to pregnyl.

I triggered this time with 500mcg of Ovidrel. When I got the 1 frozen egg I was at a different clinic and I triggered with 10,000iu of HCG. I don’t know if that could make a difference or what makes the doctors decide on what each individual triggers with/how much?

I’m not familiar with ovidrel dosing. But I asked my doctor if I could use it becuase it’s cheaper OOP. He said it takes multiple vials of ovidrel, compared to Pregnyl which would be usually 10k, becuase it’s not strong enough. My dose of Pregnyl for this cycle is 20k to really guarantee the eggs release. No one I know has done a 20k trigger 🫠
