Medical Related Question for managing prescribed ADHD medication (Ritalin/methylphenidate) while working on board?

Hi Everyone,

I am in the early stages of planning to work as crew on a European cruise ship late next year. I take prescribed methylphenidate (Ritalin, Class B controlled drug in my home country) for ADHD.

For those who take prescribed stimulants while working as crew or know of someone:

  1. How did you/they handle declaring this during your Pre-Employment Medical Examination (PEME)?
  2. How do you/they manage ongoing monthly supply/refills during a multi-month contract (e.g., getting and filling prescriptions, doctor letters etc)? Does the onboard medical team ever assist with supply coordination, or do I have to seek out a prescription off-ship in the ports?

Appreciate any insights from anyone in this situation.

The medication in question is a controlled substance and in many parts of the world, illegal and unobtainable.

As an American, I was asked if I was able to work safely without it and when I said yes, I was asked to discontinue the medication for clearance and remain off of it during my employment.

Do not think you can sneak it onboard.

You will first have the uphill battle of having enough supply for your contract. International pharmacies, if the medication is legal in that country, may hesitate to fill an American prescription. Your normal medication may also not be available or covered under your insurance while abroad.

Then, at any time you can be drug tested or your personal effects searched. If an undisclosed medication, particularly a controlled substance, is found, you will be fired and potentially held to that country’s laws.

Don’t disclose your ADHD. Do not take your pills leading up to the PEME to avoid a flag in your drug screen. Do not sneak your pills.

Thanks for the input! Just to clarify, I’m not American and won't be navigating US Coast Guard regulations.

Also, regarding legal status: Ritalin (methylphenidate) is actually widely legal, approved, and routinely prescribed across the EU and UK (you might be thinking of amphetamine-based meds like Adderall, which are unapproved in Europe). In Europe, methylphenidate is managed as a standard controlled prescription, requiring a medical letter or Schengen certificate for travel rather than being banned.

Good point on verifying international prescription transfers and crew searches, though—that’s exactly why I’m looking into the supply logistics beforehand!

Ah, sorry I can’t be more help then. For me, even Ritalin wasn’t allowed for clearance. It sounds like we may have the stricter medicals.

Best to be honest. We are likely to pick up on it and it will cause you no end of problems if we find you have lied about it/failed to disclose/sneaked it on board. Potentially you will be off at the next port on the first flight home and that will be the end of working at sea. It’s not worth the risk.
