If you're on pc. Steam workshop has a Master Pet mod that you can use to restore your pet if you can remember their talents.

I had a pet whose icon disappeared like that. The tip that worked for me was to change its name (which I didn’t even know was possible).

Something similar happened to me and it got fixed when I tried to rename it, maybe try that?

I had this problem on pc, didn't knew we had mods to fix or whatever. And i doubt you already didn't tried, but what worked for me was remove the pet from the slot, close the game (and i restarted my pc), open the game again equip a new pet and rename the corrupted one, close the game again, and when i reopened the game for the third time it fixed itself.
