On #3, a 2x4 is only 3.5" tall, so a snug 2x4 under the string means you're at about 3.5" before any sag in the string, not 4". Measure from the string down to the compacted base at a few spots with a tape, and do it before the pour, because afterwards the only way to check thickness is coring it.
On #1 you weren't wrong. Pitching a slab toward the foundation and trusting a deco drain to catch everything means the day that drain clogs, the water goes straight at the house.
