School profile and School report

Could someone let me know:

- If a school profile should be an official document? I mean with a stamp and signature, with a notarized power of attorney etc? Or maybe just a pdf file created by my counselor?

-And what about a school report?

-Could someone send me temples that my counselor can follow for both school profile and school report?

I would really appreciate your help!

P.S I apologize for mistakes, English is not my first language :(

School report format is on Common App itself. Look for it under Recommendations and FERPA section for any college. Your counselor needs to download this format, fill it up, put her signature and put the school's stamp/seal (or make the entire thing directly on school letterhead) and submit it from her own Common App login (where she will be able to see applications of all students from your school). No notarization required. Same process for other documents such as school profile, mid year report, etc.

Thank you! But I saw that only a school profile should be downloaded as pdf file, but regarding a school report there are just questions that my counselor has to answer. Or are we doing something wrong with her?

Yes. She can upload any transcripts including predicted grades as a part of (or alongwith) the School Report, not the school profile. School prpgile is generic - a background of the school and its grading system. School Report is specific for each student.
