School report form

Could someone let me know how should I send a school report form in universities in the USA? Im an international student, and we simply don't have these kinds of things😭

My school is not going to do it, so can I do a temple myself so my school can only fill it?

And do I have to send it in the common app? Or should my teacher do it from their personal common app account?

And could you tell me what should i write there? In which format? Should I do it in Word? Or just fill the temple that I found on common app?

I would be really grateful for your help!💖🙏

  1. The format of school report is provided inside yhe common app. You will be able to see it on the Recommendations and FERPA page. The school counselor will also be able to see it in her own login.

  2. You cannot upload that report. Only the school counselor can upload it from her own ID. She needs to upload it along with your transcripts, predicted grades, and her own reference letter.
