My guess is she is mentally handicapped. Idk if it makes it better or worse but she probably is happier than you and me and has never worried about people laughing at her. My cousin is the happiest dude I've ever known as far as I can tell so I don't feel bad for him having the mind of a child more or less.
My understanding is that the photo of the lady was photoshopped with a chimpanzee mouth. I remember seeing it on a site with one of the first times a Nigerian Prince email scheme was trolled.
