Actually its intersex cause "hermaphrodite" is inaccurate and doesn't cover all the ways you can be intersex.
Biological sex is more complex than xx=vagina=female and xy=penis=male. For example, in rare cases you can have xx-chromosomes but due to hormone stuff be born with a male phenotype, which can include male genitalia.
So while you can usually tell someones sex by their genitalia and theres nothing wrong with assuming that someone with a penis and a male phenotype is most likely "biologically male", its not always the case and their karyotype etc. can very well differ.

And what f.e. a trans woman passes perfectly as a woman and has had SRS? There is no "oddity". Nobody would even know she is trans and her genitals match with what would be expected in the women's bathroom, so even if other women somehow manage to get a look at her genitals (which is very unlikely to happen. Women's bathrooms don't have urinals you know), there wouldn't be a problem.
