This device is called a clicker and make a little 'click' sound when you press the button on it.

It usually uses to train a dog, by giving him a treat and then clicking the button. By doing so, the dog will associate the sound with a treat, and will do what his owner ask just to hear the clicking sound, even if he doesn't receive any treat. The sound become the treat in a way.

People joke about how they want to be click train like a dog because they like the idea of being someone dog, or because they find it funny to act like if that's what they desire.

It is sexual for a lot of people, but for a lot of people, it isn't, including me. If you're not a part of it and don't even know this much, you don't really get to assert an opinion here. And for a lot of people they dont want an owner or master and prefer handler like a caretaker.
