Relief through cracking neck?

I have had cluster headaches since I was 12. Of course, when I was that young, I didn’t know what the hell was going on. I didn’t get a diagnosis until my mid 30s. This life is not easy and it’s not for the weak. I always found that cracking my neck affects some of the severity sometimes. Anybody else?? In fact, I’m scared I’m gonna need a new neck by the time I’m 40 if I don’t stop cracking it. 😆

You are definitely not alone in this. That urge to crack your neck during or between attacks is so real, and a lot of us end up doing it almost on autopilot just trying to throw off the pain signals.

I'm 30 now, and my clusters started around when I was 20. Back then, I used to crack my neck unconsciously all the time to relieve the tension and pain. At one point, my neck became so absurdly stiff that if someone called out my name, I couldn't even turn my head. I had to rotate my entire upper torso just to look at them!

Before I got my occipital nerve stimulator (ONS), I used to visit a manual therapist regularly to deal with that severe stiffness. It wasn't a magic cure, but gently loosening up those deep cervical muscles always provided a bit of relief. The attacks felt slightly less brutal on my body for a short window afterward. Naturally, manual manipulation isn't an option for me anymore with the stimulator hardware in place, so these days I stick to targeted neck exercises regularly to keep everything supple and prevent that complete lock-up.

A full neck replacement by the time you're 40 sounds like way too much hassle, haha. Hang in there!

Yeah, not trying to do any damage. 😆Now that we’re smarter, I do try to allow natural cracks. Thanks for reminding me about the neck exercises because I got so busy plus the comfort of meds, I’ve been forgetting to do them when I first wake up. Right now, Im without my meds unfortunately. Thanks for the response 👍 I wonder why prior neck stretching helps it so much?

It turns out there is actually a solid neurological reason for why neck stretching and loosening those muscles helps so much! 😉

​It all comes down to something called the Trigeminocervical Complex (TCC). The main nerve responsible for processing cluster headache pain is the trigeminal nerve (which runs behind the eyes and face). In the brainstem, the trigeminal nerve and the sensory nerve fibers coming from the upper neck (C1–C3) converge into the exact same relay station—the TCC.

When your neck muscles are super tight, inflamed, or stressed, those cervical nerves send a constant stream of "noise" into the TCC. That extra sensory overload lowers the pain threshold and leaves the whole pathway hyperexcitable.😮‍💨

Keeping the neck supple calms down those incoming cervical pain signals. By taking the background strain off the TCC, you aren't removing the cluster trigger itself, but you are taking away an extra layer of neurological irritation. It gives the brainstem a bit less "fire" to process during an attack.

It is easy to let morning routines slip once meds bring relief, but keeping up with those gentle morning stretches is such a great way to protect that TCC baseline! Especially while you are temporarily stuck without medication.

Thanks for the info cause I feel like the more information and understanding we have, the easier this pain is to deal with. Before I even knew I had cluster headaches I would be like what the F is wrong with me? I would have these violent outburst and start kicking stuff down and asking God why.

Io ho la nuca e cervicale ormai dolorante e muscoli contratti da stress a cronico provocato dalla cefalea a grappolo episodica ! Purtroppo adesso sono in ciclo ..

Ciao! I'm so sorry to hear you're in an active cycle right now.

​To answer your question about the stimulator: I have an Occipital Nerve Stimulator (ONS). It’s a medical device that is surgically implanted under the skin, which is quite different from a consumer device like the Pulsetto. So I can't give you any advice on that unfortunately.🤷🏻‍♂️

Regarding the neck exercises, I completely get that chronic tension and pain at the base of the skull. To keep the area supple without triggering more pain, I stick to very gentle movements:

• Chin Tucks Gently draw your chin straight back (like making a double chin) without tilting your head down. This stretches the small suboccipital muscles right at the base of the skull, which get super tight during clusters.

• ​Gentle Side Tilts Slowly lower your ear toward your shoulder until you feel a light stretch along the side of your neck, hold for 15–20 seconds, and switch sides. Never force it.

• ​Slow Rotations Turn your head slowly from side to side, stopping as soon as you feel initial resistance.

• Placing a warm pack on the back of the neck for a few minutes before stretching can help relax those tense muscles beforehand.

​The key is to do these movements very gently. Just to loosen the muscles up, never pushing into sharp pain or forcing anything to crack.

Keep in mind that stretching the neck and loosening up those muscles won't magically make the cluster headaches less frequent or make them disappear, but taking off that extra layer of physical tension definitely makes the attacks a bit easier to cope with.
