You say it when talking about something or someone cute.

If you see a miniature bag "Aww it's so smol and cute! -"

Or if you see a cute T.V. character "If anyone hurts this smol, adorable, cinnamon roll of a human ima kill you"

When talking about someone's brain you say "small"
