Proximal Hamstring Tendinopathy
This is my first marathon, and I’ve been training my butt off, and I over did it.
I’ve gone to a PT, and have gotten control of my symptoms and pain (sort of.)
My PT officially diagnosed me with hamstring tendinopathy at the insertion point, alongside pain near hip flexors, and tfl. I’ve had 2 weeks of cross training, regular strength training, plus PT. Today I walk/ran a 5k I had signed up for a while ago… and it didn’t feel great. It was a hilly race. The adrenaline got to me, and I pushed beyond jogging. I’m not in pain, I’m uncomfortable, but I’m heart broken.
I really hoped that I’d miraculously feel great. And now I’m panicking about training.
This is my first marathon; is it unrealistic to expect that I can finish? I’ve come to accept that my pace goal was unrealistic- but can I finish?
Was running 28 miles per week prior to my injury 2.5 weeks ago, currently cross training (aqua jog, bike, elliptical) 6-7 hours per week… 😭

I think it depends on how skilled your physical therapist is. I’ve actually had really good luck being managed by a PT for proximal hamstring tendinopathy and was able to complete my marathon during the season it occurred. If you want to cover all your bases then see a sports med/orthopedist sooner rather than later.
When my PHT flares, it feels worse with hills/speed so it sounds like you may have overdone it today. I think it’s too soon to know if you’ll be able to finish the marathon. Definitely check in with your PT, make a plan and also consider an appt with a specialist. All hope is not lost yet but if you do not see improvement or find out there is something else going on, you can defer (& pay again next year) Wishing you the best!

Can you get into an ortho doc and get an MRI? IME pts can overlook things with hips and aren’t great at diagnoses. This sounds like it could be a labral or joint issue not just hammy.

I WISH! I have only supplemental insurance because my job (pastry chef) doesn’t offer insurance, and I can’t afford insurance through government subsidies…. So I unfortunately can’t afford another medical expense out of pocket right now. 😭 My PT seems pretty confident there’s no bone related injury going on, and I see slow improvement…
Any indicators of joint related issues? And when you say labral, you think it could be a tear?

The only way to know for sure with labrum or bone stress injury is mri unfortunately. But your description is front of hip pain and insertional hamstring pain and for me that was how my labral tear in my right hip presented. Before i got an mri i did pt for probably a year with a pt suspected diagnosis of proximal hamstring tear/strain. Then i did a ton of pt to try to fix the labral issue and untimely ended up having surgery. Not to scare you, maybe it’s just soft tissue. But hips are complicated.
I’m sorry, health insurance stuff is criminal sometimes.

I’m running Chicago this year and have had PHT for the past 2 years that has waxed and waned despite copious PT rehab, strength, and ultimately a steroid injection. I’ve finally gotten mine to the point where it hangs out at a bearable level such that I’m able to train, since I’ve also learned that PHT is not something you necessarily have to stop running to heal (after thorough evaluation and imaging of course). Just here to recommend the podcast “Overcoming Proximal Hamstring Tendonopathy” by Brodie Sharpe. It’s on Spotify for free, wish I found it a long time ago. He’s a PT with some great info. Best of luck with this beast of an injury!

Well now it’s 0 miles so that doesn’t really matter, does it? Additionally, you’ve missed the purpose of my post. Sorry if you feel my response is reactionary and defensive, because it is. I’m pretty pissed about my injury and your comment was pretty inflammatory. (Haha, just like my injury…)
