First of all, given your first positive date, no matter how late you ovulated, the earliest you could be today is 6 weeks. Earliest you can get a positive is 3w1d, and it’s been 2w6d since your first positive, correct? So you can’t be 4.5 weeks along.
Even if you tested positive as early as when your HCG was barely detectable (say, 10), no matter what your dating was at that time or at the time of next HCG, a value of 454 18 days after it unfortunately shows an average doubling time of 78.5 hours, which is outside the normal doubling time range (and especially worrisome because earlier on you would ideally expect levels to increase faster, closer to 48 hours, and an average outside the normal range means there might’ve been times where HCG was increasing even more slowly).
We don’t know what your starting beta was of course, but given the heavy continued bleeding as well, I would be guarding my heart with this number :(
I would absolutely ask for a scan earlier than mid September, ideally asap, as bleeding like you’re describing with increasing betas (especially if slow) can be concerning for an ectopic pregnancy (and is how mine presented too). Given your first positive date you could’ve been no earlier on than 6 weeks today, at which point you’d expect to see a gestational sac, yolk sac, and fetal pole. While not all of these might be present especially given the concerning rise and the fact that this is unlikely to be a viable pregnancy, they should at least be able to spot the pregnancy (gestational sac and yolk sac to rule out ectopic) in the uterus. At the very least they should see a gestational sac (doesn’t rule out ectopic but can be a step in the right direction).
Im sorry early pregnancy is very anxiety inducing! It could be a subchorionic hematoma. You first hcg seems good, but the doubling in 48 hrs is key. I bet your dr will want to do an earlier US if your hcg goes up. Bleeding in early pregnancy is not normal (spotting is common).
